THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.   

Our Responsibilities

Packer Medical Center is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, and follow the terms of this Notice currently in effect.

How We May Use and Disclose Your PHI (Examples)

We may use and disclose your PHI for:  

  • Treatment (e.g., coordinating care with other providers)  

  • Payment (e.g., billing your insurance plan)  

  • Healthcare Operations (e.g., quality assessment, training, audits)

 

We may also disclose PHI:  

  • When required by law 

  • For public health activities  

  • For health oversight activities  

  • To avert a serious threat to health or safety  

  • For workers’ compensation and certain legal proceedings (as permitted/required)

Your Rights

You have the right to:  

  • Request access to your records  

  • Request amendment of your records  

  • Request an accounting of disclosures 

  • Request restrictions on certain uses/disclosures (in limited cases)  

  • Request confidential communications 

  • Obtain a paper copy of this Notice  

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
