Our Responsibilities
Packer Medical Center is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, and follow the terms of this Notice currently in effect.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Health Information Exchange (HIE) – CRISP
Packer Medical Center participates in the Chesapeake Regional Information System for our Patients (CRISP), a regional Health Information Exchange (HIE) serving Maryland and the surrounding region.
As permitted by applicable law, health information may be shared through CRISP to facilitate timely access to health information, improve coordination of care, and assist healthcare providers and public health officials in making informed decisions regarding patient care.
CRISP may allow authorized healthcare providers involved in a patient's care to access available health information from participating healthcare organizations, including information that may be available from hospitals, healthcare providers, laboratories, pharmacies, and other authorized sources.
Patients have rights concerning the sharing of their health information through CRISP. A patient may choose to opt out of CRISP health information exchange sharing.

Maryland Prescription Drug Monitoring Program (PDMP)
Authorized healthcare providers may also access prescription information through the Maryland Prescription Drug Monitoring Program (PDMP) when permitted or required by law, including information regarding controlled dangerous substances, for purposes such as medication safety and appropriate prescribing.
Patients should understand that opting out of general CRISP health information exchange sharing does not prevent information required by law, including applicable public-health reporting and Maryland PDMP controlled-substance information, from remaining available to authorized providers as permitted or required by law.
Certain categories of specially protected health information may be subject to additional federal or state privacy requirements and will be accessed, used, or disclosed only as permitted by applicable law.
