Late Fees
Patient balances not paid within 60 days of the initial billing statement may be assessed a late fee of $20 per billing cycle. Late fees apply only to patient-responsible balances after insurance has fully processed the claim.
Administrative Fees
The following administrative fees may apply when applicable and will be disclosed in advance:
Returned/NSF payments: $30
Paper billing statements (optional): $7
Form or document completion: $25–$75
Missed appointment or late cancellation (less than 24 hours’ notice): $75
Late or administrative fees will not be assessed for balances pending insurance or for patients actively participating in an approved payment plan.
